1.1. Student Information Forms
Student information forms are used for all compulsory or academically assessed activities and with participants under the age of eighteen.
Student information forms are not legally binding documents. They establish mutual understanding and gather essential student details, including personal information, emergency contacts, medical history, and special needs. These ensure faculty and staff have accurate and up-to-date information to support students' well-being, manage emergencies, and tailor educational services to individual needs.
The following forms are to be downloaded and sent to all participants of the off-campus activity. Participants must upload or submit their completed forms before participating in any off-campus activity.
Student Information Forms folder includes:
- Emergency Contact Information Form for Travel (DOCX)
- Medical Information Form, Assessment of Emergency and Communication Plan (PDF)
- Model Release Agreement Form (PDF)
- Student Rights and Responsibilities Student Confirmation of Understanding (DOCX)
International or Multi-day Trips: Students will also be required to submit a copy of their passport and flight itinerary.
Fee Based Activities: Students required to pay an activity fee must complete the Field Exploration Fee Form (PDF), and submit to your organizing office prior to the due date.
