Waivers are used for extra-curricular activities that are not academically assessed. They should not be used for curricular or co-curricular activities.
Waivers are legal documents that release or relinquish a known right, claim, or privilege. By signing a waiver, participants waive their ability to sue the University in the event of an injury. To be legally binding and enforceable, waivers must be customized to the specific activities or situations, such as uneven terrain, stairs, transportation, and touching or teamwork activities.
The forms below are to be downloaded and sent to all participants in the off-campus activity. Participants must upload their completed forms before participating in any off-campus activity.
