1. Introduction
Off-campus activities provide meaningful and robust learning opportunities. Experiential learning opportunities also go beyond expected classroom work and regular academic activities and, therefore, come with increased inherent risk.
In the event of an injury or accident, the court would consider if the associated risks of activities were accurately represented in waivers and informed consent forms and if safety was considered in student information forms and planning and reporting documents.
To ensure a safe and equitable environment, documents must be signed prior to arrival at your destination and:
- Customized to the specific activity or risk.
- Waivers should be provided in advance and reviewed with participants prior to signing.
- Transparent of external partners or groups organizing, hosting or sponsoring the activity.
- Transparent in obvious or foreseeable risks associated with the activity and provide a description of the risks.
- Signed in the presence of a witness; parental or guardian consent must be obtained for participants under 18.
- Must be held for seven years after the end of the event or activity for which the form was signed.
1.1. Student Information Forms
Student information forms are used for all compulsory or academically assessed activities and with participants under the age of eighteen.
Student information forms are not legally binding documents. They establish mutual understanding and gather essential student details, including personal information, emergency contacts, medical history, and special needs. These ensure faculty and staff have accurate and up-to-date information to support students' well-being, manage emergencies, and tailor educational services to individual needs.
The following forms are to be downloaded and sent to all participants of the off-campus activity. Participants must upload or submit their completed forms before participating in any off-campus activity.
Student Information Forms folder includes:
- Emergency Contact Information Form for Travel (DOCX)
- Medical Information Form, Assessment of Emergency and Communication Plan (PDF)
- Model Release Agreement Form (PDF)
- Student Rights and Responsibilities Student Confirmation of Understanding (DOCX)
International or Multi-day Trips: Students will also be required to submit a copy of their passport and flight itinerary.
Fee Based Activities: Students required to pay an activity fee must complete the Field Exploration Fee Form (PDF), and submit to your organizing office prior to the due date.
1.2. Waivers
Waivers are used for extra-curricular activities that are not academically assessed. They should not be used for curricular or co-curricular activities.
Waivers are legal documents that release or relinquish a known right, claim, or privilege. By signing a waiver, participants waive their ability to sue the University in the event of an injury. To be legally binding and enforceable, waivers must be customized to the specific activities or situations, such as uneven terrain, stairs, transportation, and touching or teamwork activities.

1.3. Informed Consent
Use informed consent forms for all compulsory or academically assessed activities or with participants under the age of eighteen.
Informed consent forms are legal documents that confirm a participant's understanding of the inherent risks of a particular activity and, by signing, consent to the associated risks. To be legally binding and enforceable, informed consent forms must include the off-campus activities and details of the activities. For example, activities such as boating or hiking would include the terrain, foreseen environmental conditions, safety equipment requirements, forms of transportation or travel, etc.

1.4. Incident Preparedness and Response
Incident preparedness and response forms must be used for all multi-day and international trips to ensure transparent communication processes and avenues between all parties.
The organizing staff and faculty must complete incident preparedness and response forms to be completed and provided to the risk committee. The School Manager is responsible for submitting and keeping all incident reporting planning documents readily available. A student-facing document with pertinent information must be developed and posted in the News Bulletin on the student-facing resource. The lead staff or instructor's documents must include printed copies of the Incident Report.
