Add a feedback survey

The Feedback tool in Moodle is used to create and conduct surveys to collect feedback. Often we see feedback tools being inserted into courses for mid-point course evaluations.

For this activity you should:

1. add a feedback tool
2. add the description to the top part of the feedback tool
3. add a few questions to the tool (Tip: a simple feedback structure is "What can the instructor start doing? What can the instructor stop doing? What can the instructor continue doing? - or choose one of your own)
