Use informed consent forms for all compulsory or academically assessed activities or with participants under the age of eighteen.

Informed consent forms are legal documents that confirm a participant's understanding of the inherent risks of a particular activity and, by signing, consent to the associated risks. To be legally binding and enforceable, informed consent forms must include the off-campus activities and details of the activities. For example, activities such as boating or hiking would include the terrain, foreseen environmental conditions, safety equipment requirements, forms of transportation or travel, etc.

The forms below are to be downloaded and sent to all participants in the off-campus activity. Participants must upload their completed forms before participating in any off-campus activity.
